Basic Fabrication Requirements
To provide you with the highest standards of quality and service, we ask that you follow the basic appliance fabrication requirements listed below. Bite Registration:
- Class I/Class II Functional-single centric relation bite 1mm from first contact.
- Class II Intraoral Advancement-wax reference in advance position (use molars as reference).

Bite Recommendations:
Klearway: Open incisally and advance mandible approximately 65% or 2/3 the distance from centric occlusion to full protrusive.Herbst®*: Open bite 4-5mm incisally and advance mandible to desired protrusive position.
NAPA: Open bite 4-5mm incisally and mandibular advancement approximately 1/2 to 3/4 the distance from centric occlusion to full protrusive.
Elastomeric: Open 4-5mm incisally and mandibular advancement approximately 1/2 to 2/3 the distance from centric occlusion to full protrusive.
Elastodontics:
Upper mounted stone model and lower stone model. If brackets will remain in place, impressions must be taken without archwire in place. Please place elastics around bracket neck before impressing to minimize undercuts. Centric occlusion or centric relation bite as desired.
Fixed Appliances:
Well-defined plaster or stone work model with bands in place. Pedo-partial and Habit appliances require opposing model. Note: It is the responsibility of the clinician to prepare teeth that require banding before forwarding the models to the laboratory.
Functional Appliances:
All Functional appliances require upper and lower stone work models.
Frankel, I, II, and V: Construction bite with the mandible advanced 2-4mm and a 3mm vertical opening in the posterior segment.
Frankel III: Construction bite with edge-to-edge incisal relationship, vertical opening-incisal clearance.
Frankel IV: Construction bite with centric relation; 1mm opening in the bicuspid area.
Standard Activator: Construction bite with edge-to-edge relationship, 8-10mm posterior vertical opening.
Expansion Activator: Construction bite with edge-to-edge relationship, 7-12mm posterior vertical opening.
Bionator I "to open": Construction bite with edge-to-edge relationship, 3mm anterior vertical opening.
Bionator II "to close": Construction bite with edge-to-edge relationship, 3mm posterior vertical opening.
Bionator "to maintain": Advance mandible as desired, vertical opening 2mm interincisally and 2mm at molars.
Twin Block: Construction bite with 5-7mm advancement, 3-5mm vertical opening in pre-molar/bicuspid area.
Acrylic Splint Herbst®*: Construction bite with initial mandibular advancement 3-4mm, 2-3mm anterior vertical opening.
Cantilever Herbst®*: Initial mandibular advancement 3-4mm, no vertical opening necessary.
Positioners:
Upper mounted stone model and lower stone model.
Removable Appliances:
Plaster or stone upper and/or lower work model(s). Thickness of the base should be 7mm in the deepest portion of the palatal area. Opposing model and bite registration should be included with any case when bite plates or occlusal coverage are requested or where occlusal interference may be a concern.
Sagittal: Construction bite should represent a centric relation; 2-3mm posterior vertical opening.
Sleep Apnea:
Upper and lower stone models. Midlines should be in the same relation as it is in centric.
Splints:
Upper and lower stone models, facebow mounted models are preferred and will assure a more accurate appliance. Construction bite should represent vertical opening desired, minimum 2mm.

U.S. Customers:
Great Lakes provides postage paid U.S. Priority mailing supplies for forwarding your cases to our laboratory. All cases are returned via UPS (United Parcel Service) unless otherwise requested.U.P.S. 2-Day "On Call" service is also available for traceable forwarding of cases to the laboratory. An additional fee per box will be added and your case will automatically be returned U.P.S. 2-Day Air at no further expense. The fees charged are substantially less than actual costs and compared to other commercial laboratories.
Canadian Customers:
When forwarding cases to the laboratory, send them prepaid or billed directly to your office at:Great Lakes Orthodontics, Ltd.

Overseas Customers:
Cases can be forwarded to the laboratory freight billed directly to your office or pre paid via the courier of your choice. If you select U.P.S. Worldwide Express, our standard courier, you may choose the freight collect option on the way bill. We receive discounts due to Great Lakes' high volume usage with U.P.S. These discounts for incoming or outgoing freight are passed on to you.All freight charges, tax, and duty will be applied to your account approximately two weeks after the shipment date.
Free Shipping Supplies
We provide shipping supplies (boxes, packing materials, mailing labels) for forwarding your cases to our laboratory. Just click on the Supply Request Form link below to e-mail your request, or call us at 800-828-7626 or 716-871-1161 (Monday-Friday 8:00 a.m. to 5:00 p.m.). You may also fax your request to 716-871-0550.

To assure your case arrives safely and ready to be processed, please follow these guidelines:
- To prevent damage in transit, completely fill all spaces in shipping container with packing material.
- Print doctor's name and patient's name on model backs.
- Make sure to include a completed prescription form with the case. Please mark the due date one day before appointment.
- Wrap wax bite separately; never ship with wax bite on model.
- Seal small items in an envelope prior to placing them in the container.
- Carefully wrap model as shown, folding excess foam or packing material over the occlusion.
